Our valued Smile Care patient,
Please read the following information thoroughly to assure a pleasant and uncomplicated theatre visit as possible.

Pre- Admission:

1. Do not eat or drink six (6) hours prior to your admission to the hospital.
2. Please notify your doctor should you develop any illness 48 hours prior to the scheduled procedure.
3. A parent or legal guardian who has to give consent for a procedure should accompany patients under 18 years. Relatives or friends may not sign the consent.
4. You should report to the hospital’s admissions desk at least 45 minutes prior to the scheduled procedure time.
5. Remember to bring:
   
   • Your medical aid membership card
   • Medical aid authorization number
   • ID document
   • X-rays, test results, medication
   • Contact lens holder, if applicable
   • Favorite toy or blanket (in the case of young patients)
6. Do not bring cash or valuables such as jewelry. The hospital will not accept responsibility for any loss.
7. Firearms and weapons are not permitted in the hospital.
   

Post-Operative:

1. You should not drive a car, operate machinery or take any alcohol for at least 24 hours following anesthesia, as your judgment may still be impaired. Please arrange for a relative or friend to accompany you home.
2. Please follow all instructions regarding post-operative care and take all prescribed medication as indicated by the hospital staff.
3. Should you have any further enquiries, need any medication or suspect complications, please phone Smile Care Dental Centre on 012 663 8810.
   

Financial Arrangements:

1. Smile Care Dental Centre’s statement will be provided beforehand according to the quotation provided to you and must be settled 24 hours before the scheduled procedure time.
2. The hospital’s fees are mostly charged at medical aid rates and the account will be sent to your medical aid by their accounts department. Please note that no fees charged by the doctor or the anesthetist will appear on the statement by the clinic.
3. The anesthetist will provide a full account directly after theatre, which is also immediately payable and then, claimed back afterwards. A quotation can also be provided should you need one.
4. Please make arrangements for payment prior to your admission if you are a private patient.

Gewaardeerde Smile Care Pasiënt,
Lees asb die inligtingstuk aandagtig deur ten einde u teater prosedure so aangenaam en komplikasieloos as moontlik te maak.

Voor Opname:

1. Staak inname van alle kos en vloeistowwe ses (6) uur voor opname in   teater.
2. Lig asb die dokter in indien u enige siekte ontwikkel het 48 uur voor opname in teater.
3. Pasiënte onder 18jr oud moet deur ‘n ouer/voog vergesel word omdat daar toestemming geteken moet word vir die prosedure. Geen ander familie of vriende kan teken nie.
4. U moet ten minste 45 minute voor u prosedure aanmeld by vooropname administrasie.
5. U moet saambring:
   
   • U mediese fonds kaartjie en mediese fonds magtigingsnommer
   • ID dokumente
   • X-strale/toets uitslae/medikasie saambring soos deur u dokter voorgeskryf
   • Kontaklens houertjie (indien van toepassing)
   • Gunsteling speelding/kombers (in geval van jong pasiënte)
6. U moet verkieslik geen kontant of kosbare items soos juwele, selfone ens saambring nie. Die hospitaal aanvaar geen verantwoordelikheid vir verlore items nie.
7. Geen vuurwapens word in hospitale toegelaat nie.
   

Na Prosedure:

1. U word aangeraai om nie binne 24 uur na enige narkose te bestuur, swaar masjienerie te beheer of enige alkohol in te neem nie weens  verswakte konsentrasie. Reëlings moet asb getref word met familie of vriende om u huis toe te neem.
2. Volg asseblief alle instruksies soos deur die hospitaal personeel aan u gegee en neem die medikasie soos voorgeskryf.
3. Indien u enige navrae het, verdere medikasie verlang of enige komplikasies vermoed, skakel gerus Smile Care Dental Centre op 012 663 8810.
   

Finansiële Reëlings:

1. Smile Care Dental Centre se fooie is volgens die kwotasie wat vooraf aan u  voorsien  is. Vereffen asb u rekening ten volle die dag voor die prosedure.
2. Die hospitale se kostes is meestal teen mediese fonds fooie. U rekening word deur die hospitale se rekeningafdeling na u mediese fondse gestuur. Let daarop dat geen fooie van die dokter of  narkotiseur wat u behandel het, op die rekeningstaat van die hospitaal sal verskyn nie. 
3. Die narkotiseur se volle rekening vir die prosedure word direk na die prosedure oorhandig wat dan ook dadelik betaalbaar is. ‘n Kwotasie kan ook vooraf ontvang word daarvoor.
4. Indien u ‘n privaat pasiënt is, kan u ook vooraf reëlings met die hospitaal maak vir die vereffening van u rekening.
